The following documents are required before any athletic participation and need parent and student signatures! Please return all forms to the high school office.

Sudden Cardiac Arrest Awareness Form - Annually (2 pages):
- Parent signature, name, and date at bottom of page 2
- Student signature, name, and date at bottom of page 2
Pre-Participation Physical Form - Every other year (2 pages):
- Fill out all of page 1
- Page 2 is completed by the physician performing the exam
Parent/Student Steroid Agreement/Acknowledgement Form - Annually (1 page):
- Student name, signature, grade, and date at bottom of page
- Parent name, signature, date, and relationship to athlete at bottom of page
- 2020-2021 school year at bottom of page
Concussion Acknowledgement Form - Annually (1 page):
- Print athlete’s name at top of page
- Parent signature and date at bottom of page
- Student signature and date at bottom page
Acknowledgement of Rules Form - Annually (2 pages):
- Student’s name, date of birth, and school at the top of page 1
- Check all sports the athlete wishes to participate in at bottom of page 1
- Date, parent signature, mailing address, home/work/cell phone at bottom of page 1
