Sudden Cardiac Arrest Awareness Form - Annually (2 pages):
Parent signature, name, and date at bottom of page 2
Student signature, name, and date at bottom of page 2
Pre-Participation Physical Form - Every other year (2 pages):
Fill out all of page 1
Page 2 is completed by the physician performing the exam
Parent/Student Steroid Agreement/Acknowledgement Form - Annually (1 page):
Student name, signature, grade, and date at bottom of page
Parent name, signature, date, and relationship to athlete at bottom of page
2020-2021 school year at bottom of page
Concussion Acknowledgement Form - Annually (1 page):
Print athlete’s name at top of page
Parent signature and date at bottom of page
Student signature and date at bottom page
Acknowledgement of Rules Form - Annually (2 pages):
Student’s name, date of birth, and school at the top of page 1
Check all sports the athlete wishes to participate in at bottom of page 1
Date, parent signature, mailing address, home/work/cell phone at bottom of page 1
